How this is calculated
THE RULE IS AS SIMPLE AS HEALTH GUIDANCE GETS: KEEP YOUR WAIST TO LESS THAN HALF YOUR HEIGHT. That is a ratio under 0.5, it needs no chart, and it works in any units as long as both measurements use the same one. THE PUBLISHED BANDS, from NICE and reported identically by the British Heart Foundation: · 0.4 to 0.49 - healthy range · 0.5 to 0.59 - increased risk of health problems · 0.6 and above - highest risk The conditions the risk refers to are type 2 diabetes, high blood pressure, high cholesterol and cardiovascular disease. ⚠️ HOW YOU MEASURE MATTERS MORE THAN THE ARITHMETIC, and it is where nearly all the error comes from. Find the bottom of your ribs and the top of your hips, and put the tape around your middle at the halfway point between them - roughly just above the navel, NOT at the narrowest point and NOT where your trousers sit. Pull it snug but not tight enough to dent the skin, and breathe out naturally before reading it. Holding your stomach in, or measuring over clothes, moves the answer by more than the difference between two bands. WHY IT BEATS BMI FOR THIS PARTICULAR QUESTION: BMI uses only height and weight, so it cannot tell muscle from fat and cannot tell where fat is. Waist-to-height ratio measures central adiposity specifically - fat around the middle - which is the pattern most strongly linked to those conditions. NICE notes the ratio can be used for both sexes and all ethnicities including adults with high muscle mass, which is precisely where BMI is least reliable. ⚠️ IT IS NOT MEANT FOR EVERYONE. NICE applies these bands to people with a BMI under 35. Above that, the ratio adds little because central adiposity is already established. It is also not appropriate during pregnancy, and children have their own separate cut-offs which are not these - do not apply an adult band to a child. ⚠️ AND IT IS A SCREENING MARKER, NOT A DIAGNOSIS. It says nothing about your blood pressure, blood sugar, cholesterol, fitness or family history, all of which matter and none of which a tape measure sees. - My ratio is above 0.5. What does that actually mean?
- That you are in a band associated with increased risk at a population level - not that anything is wrong with you specifically, and not a diagnosis of anything. A ratio is a screening marker: it is cheap, quick and says nothing at all about your blood pressure, blood sugar, cholesterol, fitness or family history, every one of which matters more and none of which a tape measure can see. The sensible response is to treat it as a prompt for a conversation with a clinician who can measure the things that actually decide risk, rather than as either a verdict or something to dismiss.
- What if I am below 0.4?
- That is below the published healthy range rather than an especially good score, and it is worth mentioning to a clinician rather than celebrating. The bands start at 0.4 because the relationship between central adiposity and risk is not simply lower-is-better all the way down - being well under can reflect low body weight or loss of muscle, which carry their own concerns. The tool reports the 0.4 figure for your height for that reason, so you can see where the bottom of the range sits rather than assuming the floor is zero.
- How often should I measure?
- Rarely enough that a change means something. Waist measurement varies day to day with food, fluid and bloating by more than any real change over the same period, so measuring daily produces noise you will be tempted to interpret. Monthly, at a consistent time of day and with the same technique, gives a trend worth reading. And the trend is genuinely the useful part - a single reading places you in a band, while a direction over several months tells you whether anything is changing, which is the question you were actually asking.
